Northeast Transition Collaborative Network
The Northeast Transition Collaborative Network (NTCN) is a regional initiative that connects educators, families, professionals, and community partners to support coordinated, person-centered transition planning for youth and young adults with disabilities.
Launched in January 2025, the Northeast Transition Collaborative Network (NTCN) was created to bridge communication and collaboration gaps among school districts, service providers, young adults, families, and community organizations across the Northeast region of Massachusetts.
NTCN works to unify resources, strengthen partnerships, and provide meaningful professional development opportunities that support educators and transition professionals in the region.
WHAT WE OFFER
Monthly Webinars
Live, virtual sessions that cover a range of transition topics, including employment, education, adult services, and independent living. Webinars are designed for families, educators, and professionals, with opportunities to learn practical strategies and access shared resources.
Professional Development (PDPS)
NTCN is a DESE-approved Professional Development Provider, offering educators the opportunity to earn PDPs while engaging in relevant, high-quality training. Sessions are designed to support educators and transition professionals in strengthening their practice.
Resource Sharing
Participants gain access to curated resources, tools, and materials that support transition planning and real-world application. Resources are shared during and after sessions to help reinforce learning and support ongoing use.
Collaboration
NTCN brings together school districts, service providers, families, and community partners to strengthen communication and coordination during the transition years. This collaborative approach helps ensure more consistent, person-centered support.
