One Care/Long Term Support Services Coordination (LTSC)

One Care offers a simple way to get your MassHealth and Medicare benefits together.

With One Care, you have one plan, one card, and one person to help coordinate your care. What’s more, with One Care you are eligible to receive combined benefits and access to a large network of healthcare providers and access to a wide range of services and supports to help you safely and comfortably age in place.

Our Long Term Support Services Coordinators (LTSCs) will work with you to provide an assessment of your healthcare and long-term services and supports needs, then coordinate and manage those services in alignment with your One Care care team.

Program Eligibility:

• Age 21-64
• Eligible for MassHealth Standard or MassHealth CommonHealth
• Live at home with support services or in a long-term care facility
