Northeast Clinical Services

Northeast Clinical Services delivers in-home health care services in the two primary programs. These two services are Continuous Care Nursing and Skilled Intermittent Care. If you would like to learn more, please read below or simply call our main number 978-646-5200.

Continuous Care Nursing

Continuous Care Nursing, sometimes called Private Duty Nursing, is our primary service. In the CCN program we provide in home nursing care in blocks of time ranging from two hours per day all the way up to 24 hours per day. Our nurses generally work six to twelve hour shifts but the staffing is highly specific to patient diagnosis, physician orders, patient and family desire and insurance approval. As a general rule it is the insurers providing coverage for continuous care nursing services that determine the medical necessity as well as the appropriate number of CCN hours per week to be provided. This is almost always done in advance of any service being delivered. After approval is established, the patient and family may have some flexibility around the hours per day or time of day or night, but this is not always the case. What dictates the care regimen is the patient’s assessed and determined need and the specific treatment required.

Nurses in our CCN program care for very sick children and adults. As a typical provider of this service most of our patients are children under the age of 18 or young adults ranging from 18 years to mid-twenties. The diagnoses vary widely from disorders like Leukodystrophy, Cerebral Palsy, Muscular Dystrophy, Multiple Sclerosis, and ALS to cardiac disease and accident related disabilities. Our nurses are well trained to care for all types of diseases and disabilities and we assure that a nurse’s experience and skill set matches a patient’s need. We also understand that a comfortable personality fit between nurses and families is a true advantage when dealing with unique and demanding home care situations and we work hard to achieve a high level of compatibility.

Skilled Intermittent Care

Skilled Intermittent Care services consists of traditional VNA style home visits, usually no more than 30 minutes in length, and can be nursing, physical therapy, occupational therapy, speech pathology, medical social work or home health aide. The visits are not only short in length but also usually delivered in a frequency of no more than two to three times per week.  Generally, these services are delivered to an acute patient or one recently discharged from a hospital. The care tends to be short term and delivered in 60 days or less. Longer periods are possible depending on the illness but in general the care is designed to meet a skilled intermittent need over a short period of time.

Skilled Intermittent Care is provided by experienced caregivers and is provided in collaboration with the patient’s physician and specific to what the physician has ordered for the patient. Most of our Skilled Intermittent Care is nursing care although we do offer services in all the other home health care disciplines as well, including physical therapy, occupational therapy and speech pathology. Skilled Intermittent Care is provided to both the pediatric and adult populations throughout our service area.
