Continuous Skilled Nursing
Continuous Skilled Nursing (CSN), also known as Private Duty Nursing, is our core service, dedicated to providing exceptional in-home care for those in need. We offer nursing support in flexible time blocks ranging from 2 to 24 hours per day, ensuring care that fits seamlessly into your life.
Our nurses typically work 4 to 12-hour shifts, with schedules carefully designed to meet the unique requirements of each patient. Plans are developed based on physician orders, family preferences, and insurance approvals, so care is tailored and thorough. Insurance providers assess medical necessity and determine the number of approved CSN hours before services begin. Although families may have some flexibility in adjusting schedules, every care plan prioritizes the patient’s specific needs.
We specialize in supporting children and adults who face complex health challenges, including conditions such as Leukodystrophy, Cerebral Palsy, Muscular Dystrophy, Multiple Sclerosis, ALS, cardiac conditions, and disabilities stemming from accidents. Our nurses are selected based on their expertise, experience, and compatibility with each patient and family. This thoughtful approach ensures not only high-quality medical care but also a compassionate and comfortable connection that fosters trust and support.

Complex Care Assistance
Complex Care Assistant (CCA) services are a thoughtful new option designed to provide additional support for Community Case Management (CCM) members and offer family caregivers the opportunity to be compensated for delivering specialized care to loved ones with medical complexities. These services work alongside Continuous Skilled Nursing (CSN) care, offering complementary assistance rather than replacing it.
CCAs are trained to perform more advanced tasks than Home Health Aides (HHAs), ensuring patients receive care tailored to their specific needs. Provided through a CSN agency, these services are carefully designed to help individuals facing significant health challenges while also supporting the families who care for them. The aim is to offer compassionate, comprehensive care and to empower families in their caregiving journey.
Physical Therapy, Intermittent Skilled Visits, Medical Social Work
We offer select services in Physical Therapy, Intermittent Skilled Visits, and Medical Social Work exclusively to our existing patients, ensuring their ongoing care needs are supported.
