Make a Referral

To make a patient referral to the Continuous Care Nursing program or the Skilled Intermittent Care program, please call our main phone number 978-646-5200. You will be immediately connected to one of our office nurses or patient care coordinators who can then answer your questions and take the needed information to arrange for a prompt in-home assessment.

Northeast Clinical Services accepts referrals 24 hours a day, seven days a week. After normal business hours, our main phone number can be used to reach our answering service which will connect you with on-call personnel to assist you in the referral process.
