Navigating the Transition Together: Exploring DDS Adult Services

This presentation will help begin building the bridge between school-based services and adult DDS services and provides essential information for families about adult DDS supports.

What Participants Will Learn

  • Understand the DDS eligibility process and the steps involved in accessing adult services
  • Navigate the transition from school-based supports to adult DDS services and learn what to expect during the process
  • Identify the range of DDS supports and services available to help individuals and families achieve their goals in adulthood

Once registered, participants will receive a Zoom link to join the webinar.

PDP Information (Educators)

Are you an educator looking to claim PDPs?
The Northeast Transition Collaborative Network (NTCN) is a DESE-approved PDP provider.

To be eligible for PDPs under Special Education – Transition, participants must:

  • Attend NTCN sessions totaling 10 hours
  • Complete a post-session feedback survey
  • Complete a post-session knowledge assessment and score 80% or higher

A certificate of attendance will be sent after completion of the post-session knowledge assessment.
NTCN tracks participant hours and will issue a certificate of completion once the 10-hour requirement and all follow-up components are met.
