Nursing Case Management

The Northeast Arc’s Health Department employs Nurse Case Managers. The role of the Nurse Case Managers is to assist individuals, family members and staff in understanding particular health risks, treatment options and to provide training and education around health related topics. Nurse Case Managers also interface with community health care providers to ensure that appropriate treatment is being provided. Many individuals have multiple health care issues resulting in a variety of health care practitioners. Nursing Case Management staff coordinate care by serving as liaisons between providers.

The Northeast Arc also provides 24-hour access to an on-call nurse. This safeguard is in place to provide assistance and guidance to individuals and staff at all times. All hospitalizations and discharges, including emergency room visits, are coordinated through the on-call Nurse.
